When diffuse, NSOI may diffusely involve the orbital fatty tissues. Dacryoadenitis is an inflammation of the lacrimal gland. The initial symptoms typically consist of discomfort in the region of the lacrimal gland (i.e., the superotemporal orbit) and occasionally, pain. 1 However, severe cases may be characterized by swelling, tenderness, heat and redness in the tissues surrounding these glands. Orbital cellulitis most commonly refers to an acute spread of infection into the eye socket from either extension from periorbital structures (most commonly the adjacent ethmoid or frontal sinuses (90%), skin, dacryocystitis, dental infection, intracranial infection), exogenous causes (trauma, foreign bodies, post-surgical), intraorbital infection (endopthalmitis, dacryoadenitis), or from spread through the blood (bacteremia with septic emboli).

Rapid onset, hours to days. Disease Dacryocystitis is inflammation of the lacrimal sac which typically occurs secondarily to obstruction within the nasolacrimal duct and the resultant backup and stagnation of tears within the lacrimal sac. When localized, inflammation can affect the extraocular muscles (orbital myositis), lacrimal gland (dacryoadenitis), sclera (scleritis), uvea (uveitis), and the superior orbital fissure and cavernous sinus (Tolosa-Hunt syndrome). Others include periscleritis, perineuritis, and mass.

The lacrimal gland is located superotemporally to the globe, within the extraconal orbital fat (see Figure 1). Pleomorphic adenoma of the lacrimal gland (PALG), also known as benign mixed tumor, is a rare tumor of the lacrimal gland.[1] PALG typically presents with unilateral painless proptosis.

In children, systemic signs are present in up to 50% of patients. Headache, emesis, anorexia, lethargy, and fever are the most common systemic signs. Dacryoadenitis may be infectious or non-infectious in etiology (non-specific orbital inflammation of the lacrimal gland), present in a similar fashion and may be difficult to distinguish from one another in the early phase.
